The Indiana Department of Homeland Security is distributing a flood damage questionnaire to counties hit hard by the flooding. It's the first step to try to get federal dollars to help homeowners.


Kosciusko County

Questionnaires can also be picked up at the following locations:

  • Kosciusko County Justice Building, 121 N. Lake St., Warsaw, east entrance
  • Warsaw Police Department, 2191 E. Fort Wayne St., Warsaw
  • Warsaw City Hall, 302 E. Market St., Warsaw
  • Winona Lake Clerk's Office, 1310 Park Avenue, Winona Lake
  • North Webster Clerk's Office, 144 S. Main St., North Webster
  • Pierceton Clerk's Office, 105 Walnut St., Pierceton
Questionnaires should be completed as soon as possible and returned to the Kosciusko County Emergency Management at 121 N. Lake St., Warsaw, IN 46580 or faxed to 574-371-2611.
